Please fill out this application anytime you would like to use Foothills Church for a ministry or special event. Filling out this application form does not guaranty your usage. General guidelines: 1) Events must be scheduled at least eight weeks in advance. This does not apply to memorial services and some FH ministry use. 2) The following dates are unavailable: Holy Week and Easter, Christmas Eve and Christmas Day, all days between December 25 and January 2. 3) Use of sanctuary on a Saturday is limited to before 7:00pm (including clean up of event). Set-up and Stage Usage

I would like to use your round Tables (seat 8; 20 tables total) I would like to use your 8 ft rectangular tables (seat 10; 6 tables total) I would like to use the stage I would like to use your podium on stage I would like to use the Piano on the stage I will not be using your tables, stage, podium or piano

Technical details

Microphone(s) will be needed Sound Tech ($50 per 2 hrs) Projectionist ($50 per 2 hrs) I have DVD, CD, PowerPoint, Keynote or other media (must be made available to projectionist 2 weeks prior to event) I will not be using your sound, A/V, or any technical equipment

Fees and Other

I understand that Rental Fees may be charged for use of Foothills facility (up to $800) I understand that custodial fees may be applied to my use of Foothills facility ($100 base fee) I understand that I will be expected to pay for a sound tech and/or projectionist if I need them at the listed rate I understand that a refundable security deposit applies to all events held at Foothills Facility ($250 min)
