Start the Party VBS

July 8-11 6-8pm | Registration for children ages 4yrs. thru 5th grade (in fall 2024) to attend Start the Party VBS at Foothills Church July 8-11th (plus Family Night July 12th!).
Cost: $20 1st child, $15 for each child thereafter, no more than $50 per family (scholarships available)

During "Start the Party" VBS we will use social media to share what kids are doing each night.  When posting photos, we may use children's first name (not last) in the post. You may follow "Start the Party" VBS using the hastags #starttheparty and #cmfoothills.  Please let us know what is appropriate for your family in the box below.

As a parent or guardian, I/we hereby give my/our permission for the above-named child(ren), my/our minor child(ren), to attend VBS organized and sponsored by Foothills Church, Stayton, Oregon.  July 8-11, 2024 @ Foothills Church.  I also understand that my child(ren)needs to abide by the rules of the event leadership during the course of the event.
I am aware in signing this document for the above-mentioned child(ren) participation in Foothills Church's VBS, that certain elements of these activities are physically and emotionally demanding.  Furthermore I/we understand and acknowledge the potiential risks and dangers involved in the participation of the event and I have voluntarily authorized their participation in this event and treatment of minor injury and/or admission to a nearby medical treatment center if necessary.
In consideration of being allowed to participate in Foothills Church's VBS, I/we hearby personally assume for my/our minor child, all risks in connection with said program for any injuries or damages which may occur to my child as participants and fully and forever release the church's, it's employees, affliated organizations and volunteer staff from any and all claims, demands, damages, rights of action or causes for action, present or future, whther the same be know, anticipated or unanticpated, resulting from or arising out of the participants's commencment of the activity or use of the facilities, equipment and property of Foothills Church except in the case of negligence.

I do, on my behalf of my/our child(ren), agree to identify and withhold Foothills Church and the employees and volunteer staff thereof any liability and expenses for personal or property damage or injury not casued by their negligent actions.

My signature on this document is also intended to bind my successors, heirs, reresentatives, adminstrators and assigns.
